Family Caregiver Support Program
The U.S. Congress
authorized the National Family Caregiver Support Program administered
under the Older Americans Act. The program helps family caregivers of
loved ones who are ill or have disabilities. It addresses family needs
from planning and early intervention, to assistance throughout the various
stages of need and long-term care. Consultation is provided by trained
professionals through home and office visits, and telephone.
TRANSITIONS
is a resource center for people who are taking care of a loved one, either
temporarily or on a long-term basis. Information and services are provided
free of charge. All contacts are private and confidential.
You qualify for the
program if:
- Your loved one
is age 60 or over
- You or your loved
one lives in Lassen, Modoc, Shasta, Siskiyou, or Trinity County
- You provided care
for a loved one: spouse, parent, friend, or grandparent
TRANSITIONS -
A Family Caregiver Support Program offers a full complement of family
services designed to assist caregivers of older individuals that include:
- Specialized
Information - Providing a central source of information about caregiving,
care options, long-term care and services for families and professionals
caring for older individuals. A comprehensive resource lending library
is available.
- Caregiver Counseling
- Consultation is provided by trained professionals through home
visits, telephone and office visits. Caregiver Advocates assess problems
and needs, explore care options and provide information to caregivers
and family members. Short-term counseling is provided through licensed
counselors for caregivers and family members who need assistance with
problem identification and solution exploration.
- Support Groups
- Monthly meetings in a supportive atmosphere to share experiences and
ideas to ease the stress of caregiving.
- Caregiver and
Professional Training - Workshops are provided on a variety of topics
for caregivers, family members and professionals. Topics may include
stress management, legal and financial issues, treatment and long-term
care planning.
- Legal Consultation
- Personal consultation with an attorney on financial planning,
placement issues, conservatorship and other related matters.
- Respite Care
- Subsidies for in-home care or day care to assist families
in caring for a loved one at home.
- Home Modification
- Subsidies for limited home modification, such as installation
of grab bars.
- Placement Services
- Assist caregivers in securing appropriate nursing home living arrangements.
